Familiar routines, meaningful relationships and compassionate support
We provide person-centred home care for adults living with dementia. We take time to understand each person’s life history, communication style, abilities, preferences and routines so that care feels familiar, respectful and reassuring.
We work collaboratively with the individual, their family, representatives and relevant professionals, where appropriate and with consent. Our approach focuses on the person rather than the diagnosis and promotes dignity, inclusion, comfort and the greatest possible independence.
Dementia care may include:
- Support to maintain familiar daily routines
- Personal care provided at the person’s pace
- Medication, nutrition and hydration support
- Gentle prompts, reassurance and emotional support
- Meaningful activities based on the person’s interests and abilities
- Support with orientation, communication and decision-making
- Assistance to remain active and connected with family and the community
- Observation of wellbeing and prompt reporting of changes or concerns
- Support to maintain a safe and comfortable home environment
