Helping you return home safely and regain confidence
Returning home after a hospital stay, illness or change in health can feel challenging. We provide short-term, outcome-focused support to help adults settle safely at home, rebuild confidence and regain or maintain everyday skills.
Support may include:
- Assistance with personal care and dressing
- Support with mobility and safe movement around the home
- Medication support in line with discharge instructions and the agreed care plan
- Meal preparation and support with eating, drinking and hydration, including monitoring intake where required by the care plan
- Shopping for groceries or essential items during the recovery period
- Light housekeeping, laundry, changing bed linen and essential household tasks that help maintain a safe environment
- Assistance with follow-up appointments and community activities
- Encouragement to practise agreed daily living tasks and rebuild independence
- Monitoring wellbeing and reporting changes or concerns to the appropriate person or professional
- Liaison with family members and relevant health or social care professionals, with consent
